Troopers from the Delaware Post of the Ohio State Highway Patrol are investigating a two-vehicle crash that left one dead Sunday afternoon in Concord Township.

The post reports that the crash occurred at approximately 6 p.m. Sunday at the intersection of Moore Road and Concord Road when a 2007 Honda Civic, driven by Angel Josue Alcazar Velazquez, 27, of Delaware, was traveling north on Concord Road. Troopers report the vehicle failed to yield at a stop sign and was struck by a westbound 2019 Ford F-250 driven by Larry Coffing, 70, of Piqua.

Both vehicles then traveled off the northwest corner of the intersection.

The post reports a passenger of the Honda was pronounced deceased at the scene but did not release the name Sunday as troopers were still notifying next of kin.

Alcazar Velazquez sustained serious injuries and was transported to Riverside Methodist Hospital. Coffing and his passenger sustained minor injuries and were also transported to Riverside, the post reported Sunday.

Troopers reported that all drivers and passengers were wearing their safety belts at the time of the crash and said alcohol does appear to be a contributing factor in the crash.

The patrol was assisted at the scene by Liberty Township Fire Department and Concord Township Fire Department.

The crash remains under investigation.
